When you sign up for passport expediting services, they will send you an email, or give you a link to all the forms you will need to have filled out. They will also provide detailed directions and what necessary documents you will need to procure prior to submitting your application.

Necessary documents include:

  • Primary proof of Citizenship – This is typically a certified copy of your birth certificate.
  • Proof of Identification – This is usually a copy of your driver’s license or state or military issued ID card.
  • Two Passport Photos – 2″x2″ Identical photos to include on your passport

The main reason most passport applications are delayed is because of improper or unacceptable photos.  To avoid delays, have your passport photo taken professionally.  Most photolabs are up to date with current passport photo requirements, and can take and process your passport photo in as little as 10 minutes.

New Passport (Form DS11)

You must use this form if:

  • You have never had a U.S. passport; or
  • You are less than 16 years old; or
  • If your passport was lost or stolen. (In which case, fill out Form DS-64, in addition to DS11)

Lost, Stolen or Damaged Passport

If your passport is Lost, Stolen or Damaged, you must fill out both the DS-11 and DS-64 forms:

You must use this form if you meet all of the following criteria:

  • Your prior passport was issued when you were 16 or older.
  • Your prior passport was issued within 15 years.
  • Your prior passport is within your possession.
  • Your prior passport is not mutilated or damaged
Name Change (Form DS5504) and Passport Pages (Form DS4085)

You must use one of these forms if:
  • You need to change the name on your passport (Form DS5504 – Name Change)
    - This form only applies if your passport is less than a year old

You can use the Statement of Consent Regarding a Minor (DS-3053) form if:
